Development of an exemplar care and referral pathway

Cancer Australia is seeking to procure a supplier to develop a care and referral pathway for shared follow-up and survivorship care for early breast cancer that embeds the specialist/GP shared model of follow-up and survivorship care for early breast cancer into the pathway, aligns with the Optimal care pathway for women with breast cancer and aligns with current evidence-based guidance on follow-up and survivorship care for early breast cancer. The pathway is intended as an exemplar evidence-based care and referral pathway for shared follow-up and survivorship care for early breast cancer that can be adapted at the local level by primary health networks.
